Portia Hudson, 58, is facing charges for the death of St. Xavier junior Kevin Le.
She has been cited for vehicular manslaughter, a second-degree misdemeanor, and disobeying a traffic control device, a minor misdemeanor.
The crash occurred on Dixie Highway near Boymel Drive on Sept. 8.
The report says, as her car approached the intersection, the light turned red however Hudson failed to stop.
Sixteen-year-old Kevin Le of Liberty Township stepped into the intersection and the carhit him. He struck the car's windshield, rolled over the top, and landed in the road.
Hudson will be arraigned Sept. 30 at 9 a.m. in the Fairfield Municipal Court.
